Registered Nurse/Licensed Vocational Nurse (RN/LVN) - A nurse may provide: medication
management and instruction, skin and wound care, pain management, diabetic teaching and
at each visit monitor blood pressure and basic vital signs.

Physical Therapy (PT) - Physical Therapists work to improve mobility, regain range of motion and improve balance. Our Physical Therapists have designed a “Better Balance for Life”™ specialty program as well.

Occupational Therapy (OT) - Occupational Therapists focus on daily living skills such as bathing, toileting, meal preparation, or driving. The goal is to help individuals return to the daily living activities he or she did before an illness or surgery. OT’s can also recommend home modifications for safety, such as grab bars or ramps.

Speech Therapy (SLP) - Speech Language Pathologists provide rehabilitation in the areas of improving memory, communication skills, high level problem solving and swallowing disorders.

Home Health Aide (HHA) - The Home Health Aide works under the direct supervision of the nurse or therapist and provides assistance with all self care activities such as bathing.

Social Worker (MSW) - MSW’s assist the patient, family or caregiver in resolving social or emotional problems by incorporating community resources or counseling.

Registered Dietitian (RD) - Our Registered Dietitians are an important part of our inter-disciplinary team. They provide guidance to some families/caregivers and patients on nutritional principles, dietary plans and end of life food selections. All plans are adapted to the individual’s illness and reevaluated as nutritional needs change.
